Here is a dramatization of the type of mistakes that go on in our hospitals every day. This is why it is so important to have family accompanying patients during their hospital visits no matter how minor the operation.

I do agree that any patient who is having a procedure done in a hospital should have family present, there are some very visable mistakes present in the dramatization - To begin with almost all drug mix ups occur with only one patient - all hospitals have what are called "look alike, sound alike drugs" the name pretty wells explains it - it is a list of drugs that either look like or sound like something else and all staff are to be trained on these drugs (per JCAHO), also per JCAHO any orders that the dr themselves do not write on the chart are to have a "read back" completed - basically the nurse (only an RN can do this also) who takes the verbal order (whether the order is given in person or over the phone) must write the order down in the presence of another nurse (once again only RN) and then they must read the order back to the dr who must say "correct" then both nurses will sign off on the order. The same goes for called in lab results. JCAHO started cracking down on hospitals in alot of ways - the read backs, the look alike, sound alike drug lists, and also they have started to monitor the nurse to patient ratio because of the medical mistakes. It is a sad thing that can happen but this type of problem is beginning to happen less and less.
