I find it hard to believe that in this day and age of the electronic world, that we still have to give a doctor a full medical history each time we have to see a specialist. There is no prior knowledge and honestly you could completely make up your history and no one would know any different! Other countries have the PCP or GP hold the records. All additions, or notes, from specialists are returned to the file that the GP holds, so that is a complete record of your health issues to date. And when you go to a new place the doc has seen this, and it is not relying on your to remember every drug and condition you have ever had. I find the lack of cohesion frustrating. Is it ever going to change? |

I agree completely. It would be so much easier for all involved if medical information was all in one place. Also, I think it would save everyone involved a lot of money. |

I don't know. There are so many computer hackers out there. Wouldn't you be afraid the wrong people could get hold of your personal information? |

I would imagine that everyone's medical history is online someplace. I'd guess that it's easier for new doctors to talk to patients rather than just read a folder. In one way filling out the form and going over it with the doctor gives us the only chance we get to talk at all to a doctor who is new to us. As usual the patient is caught in the middle. If all of our information is online for any doctor to look at, then anyone can easily get in to see our very personal information. If it's not online anywhere we have to fill out those forms. I'd rather fill out that form. It also gives us something to do as we sit waiting for hours to see the doctor. |

I also agree. Anybody could become a pharmacist, even a drug dealer. I mean nobody ever completely knows a person! Someone could look like a great doctor or pharmacist and seem trustworthy, but another person could see that same trustworthy doctor you see, as their drug dealer! |

Generally, if you have signed the proper consent forms, the doctor will have all the information in your history. However, they don't have time to review your whole history each time you visit so it becomes your responsibility to make sure they are updated. |
